Mom, Dad, this page is for you.
While we hope that you will enjoy all aspects of the Youth Web Site, this page is where you should
be able to find out all the who, what, where, when, and why.
You will get a heads up on upcoming events, and meal duty schedules. We hope to keep things as
up to date as possible, but if you have any ideas about something to add, or if you know of
information that is missing, please let us know. Check out our latest calendar for every week

Emergency Medical Form: Standard medical form required for all trips. Please update your medical
form when needed or once a year and give to the youth minister.
Generic Permission Slip: Standard permission form required for all trips and special events.
Please fill out and have your youth bring to trips or special events.
Prescription Medication Information: Asks what medications and dosage your youth might be
taking at the time of each event.
